This individual is supervised by the Social Work Supervisor: Madison Clinic.<BR><BR>Primary Job Responsibilities:<BR>* Provide HIV/AIDS intake and case management to adult patients served in Madison Clinic.<BR>* Conduct comprehensive assessments of psychosocial, financial, medical support and environmental needs.<BR>* Develop individualized case management service plans to assist the hospitalized and ambulatory care patient to obtain needed medical, social, financial, legal and home care services, housing, psychiatric and drug treatment and emotional support.<BR>* Implement, coordinate and monitor the service plan.<BR>* Attend case consultations.<BR>* Have knowledge of physiological components of HIV and AIDS related illnesses, basic medical terminology and procedures.<BR>* Provide social work coverage for the Madison Clinic as scheduled to coordinate social work services with medical, psychiatric, nursing and front desk staff and manage information and referral requests.<BR>* Accept referrals of HIV+ individuals who are not yet engaged in care but are considering receiving primary care through Madison Clinic. Work with these individuals to assist them to link to care and other services.<BR>Maintain a strong social work role in assigned service area and collaboratively provide patient care with other disciplines in this area.<BR>* H </font></td>
