<td align="left"><font style="font-size: 100%;">Job Description:<BR><BR>Social Worker<BR>Emergency Depatment<BR>Seattle, WA<BR>Part Time (30 hours/week - Sunday, Monday, Tuesday & every other Saturday)<BR>Night Shift (11:00pm - 7:30am)<BR>Competitive Salary + DOE<BR><BR>The Social Worker provides social work interventions for priority Medical Center patient and family needs.<BR><BR>1. Coordinates discharge plans and/or care plans for in/outpatients, including developing plans for ongoing treatment through services and agencies.<BR><BR>2. Performs a psycho/social assessment for continuity of care planning for in/outpatients.<BR><BR>3. Provides short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability.<BR><BR>4. Performs psychiatric and chemical dependency evals in coordination with psychiatry dept.<BR><BR>4. Participates as a member of an interdisciplinary team to develop a coordinated care plan for patients.<BR><BR>5. Participates in quality assurance and utilization review to enhance patient satisfaction.<BR><BR>6. Participates in informal and formal consultations; provide, present, and participate in support and training; arrange speakers and develop materials and programs.<BR><BR>7.
