Coordinates discharge plans and/or care plans for in/outpatients, including developing plans for ongoing treatment through services and agencies.<BR><BR>2. Performs a psycho/social assessment for continuity of care planning for in/outpatients.<BR><BR>3. Provides short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability.<BR><BR>4. Performs psychiatric and chemical dependency evals in coordination with psychiatry dept.<BR><BR>4. Participates as a member of an interdisciplinary team to develop a coordinated care plan for patients.<BR><BR>5. Participates in quality assurance and utilization review to enhance patient satisfaction.<BR><BR>6. Participates in informal and formal consultations; provide, present, and participate in support and training; arrange speakers and develop materials and programs.<BR><BR>7.
