The Social Work Assistant II provides vital services in support of our Inpatient Medical/Surgical Services.<BR><BR>Job Responsibilities:<BR>* Manages Allscripts work flow including DPC task list, referrals and follow-up in a timely fashion<BR><BR>* Pilots software and other system upgrades as requested by the unit and/or department<BR><BR>* Serves as hub for problem-solving when software or other systems outages<BR><BR>* Provides a variety of program support duties including answering Center telephones, copying, faxing and compiling documents.<BR><BR>* Maintains updated Washington State facility resource lists by level of care, including populations served<BR><BR>* Assists with office organization to ensure smooth functioning of the unit and it?s processes<BR><BR>* Liaisons with community agencies/resources<BR><BR>* Documents placement outcomes in patient medical records<BR><BR>* Ensures confidentiality of all documents and maintain records accordingly<BR><BR>* Participate regularly in unit and department staff meetings<BR><BR>* Maintain positive relationship with care facilities in the community<BR><BR>* Assist community based providers with customer service<BR><BR>* Provide knowledge of community resources relevant to the discharge and home care needs of patients.<BR>This position provides opportunitiesto work in a fast-paced, challenging, diverse and rewarding environment.
