               <td align="left"><font style="font-size: 100%;">Case Manager<BR>Department: Case Management<BR>Schedule: Part Time<BR>Shift: Day<BR>Hours: 48 hours every 2 weeks<BR>Job Details: <BR>* The rehabilitation Case Manager performs activities and duties related to recruitment, in-service education, preadmission evaluation, and admission of clients to services along the rehabilitation continuum of care. Licensure as a registered nurse, preferably with a bachelor?s degree in nursing from an accredited school . A minimum of 2 years of clinical experience; experience in the rehabilitation of chronically or catastrophically ill or injured individuals. Minimum of 1 year case management experience, preferred. Certification in rehabilitation nursing or a related specialty or ability to attain within 2 years of hire. Demonstrated expertise in analysis, decision making, time management, oral and written communication, and computer use. Familiarity with the resources available for use in accessing regulations and the parameters of third-party reimbursement.
