               <td align="left"><font style="font-size: 100%;">Title: Registered Nurse Case Manager - Home Health<BR>Location: Washington-Spokane<BR>Job Number:_17032<BR>Providence  is calling a Home Health Registered Nurse Case Manager to Providence Visiting Nurses Association (VNA) Home Health in Spokane, WA.<BR><BR>We are seeking an experienced, independent and compassionate RN to provide quality care to patients and their families in the home.<BR><BR>This position will work throughout Spokane County and Lower Stevens County.<BR><BR>In this position you will:<BR>* Drive throughout your work day to see patients in private residences, assisted living, and skilled nursing facilities within a designated region<BR>* Assess, plan, implement and evaluate nursing care for the patient utilizing the interdisciplinary care team<BR>* Demonstrate critical thinking skills, decisive judgment and the ability to work with minimal supervision<BR>* Utilize software programs for the maintenance of patient records and communication with team members<BR><BR>Qualifications:<BR><BR><BR>Required qualifications for this position include:<BR>* Graduate of an accredited school of nursing<BR>* Current Registered Nurse (RN) licensure<BR>* Current driver?s license and insured, reliable vehicle<BR>* Current CPR certification<BR>Preferred qualifications for this position include:<BR>* Bachelor?s Degree in Nursing (BSN)<BR>* One (1) year nursing experience in home health or hospice; acute environment such as medical/surgical, orthopedic; subacute or long term care<BR>* Strong computer keyboarding skills and experience working with an Electronic Medical Record (EMR) system <BR>* Experience with Microsoft Office software programs including Outlook, Word, and/or Excel<BR>About the ministry you will serve.<BR>Providence VNA Home Health is a not-for-profit, home health ministry serving patients in their places of residence in the greater Spokane community of Washington state. We provide health care for patients of all ages who have a temporary condition such as post-surgical care, or are recovering from or living with a chronic condition.
