<td align="left"><font style="font-size: 100%;">The role of the ARNP at Lower Valley Hospice and Palliative Care is an instrumental position supporting the organizational priorities and goals associated with delivering cost-effective, timely and high quality healthcare services.<BR><BR>The ARNP:<BR><BR>1. Is responsible for providing medical care within the scope of family medicine, healthcare education and care assessments/coordination.<BR>2. Is responsible for ensuring that the medical care provided is consistent with the organization's standards for quality care, and that the appropriate level of care is delivered in a timely manner.<BR>3. Provides expertise and medical services in the following areas:<BR><BR>• Patient assessment and evaluation scoring,<BR>• Delivery of high quality care and<BR>• High quality patient encounter documentation.<BR>ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES<BR><BR>1. Provides timely initial patient and family assessments for palliative care consults.<BR>2. Conducts and facilitates the palliative care consultation meeting with the palliative care team in collaboration with the treating medical team.<BR>3. Participates in the assessment, planning, implementation and evaluation of palliative care interventions, including acute and chronic pain and symptom management.<BR>4. Provides patient and/or family with health education, counseling and referrals to other health professionals and community resources, as appropriate.<BR>5. Evaluates quality outcomes for the PCCS through participation in the development of palliative care quality indicators.<BR>6. Works with other staff to develop the palliative care program, including clinical practice guidelines, standards, policies and procedures in support of quality care.<BR>7. Assists with the creation and support of marketing and community education program resources.<BR>8. Participates in interdisciplinary group meetings.<BR>9. Consults with the Hospice Medical Director and informing primary physicians of services provided.<BR>POSITION QUALIFICATIONS<BR><BR>1. Current and valid state Registered Nurse licensure or PA-C license.<BR>2. Master's Degree from an accredited Nurse Practitioner Program or PA-C certification from an accredited PA Program.<BR>3. Certified ARNP/PA-C with prescriptive authority, including controlled substances.<BR>4. Minimum of six (6) months experience working in palliative care/hospice or two (2) years minimum experience working in hospital-based oncology or equivalent two (2) year experience working with seriously ill patients in an institutional setting.<BR>CPR certification.<BR>6. Current state driver's license and automobile insurance.<BR>7. Visual acuity to carry out tasks. Hearing capability to sense body sounds. Sense of touch to assess by palpation. Ability to smell common odors related to disease process.<BR>8. Proven excellence working in an interdisciplinary collaborative team environment.<BR>9. Effective interpersonal skills, leadership skills and clinical expertise in the care of patients with life-limiting illness.<BR>10. Training and experience in pain and symptom management preferred.<BR>11. Board certification as an advanced practice hospice and palliative care nurse or PA-C by a national certifying body and oncology experience preferred.<BR>12. Ability to reason, deuce and carry out complex multiple step processes.</font></td>
