               <td align="left"><font style="font-size: 100%;">Title: Clinical Pharmacy Specialist<BR>Location: Washington-Walla Walla<BR>Job Number:_15206<BR>Providence is calling a Clinical Pharmacy Specialist to Providence St. Mary Medical Center in Walla Walla, WA.<BR>The Clinical Pharmacy Specialist provides coordinated, comprehensive, high quality care to patients in the clinics and medical center.<BR><BR>In this position you will:<BR><BR>* <BR>Give direct patient care by providing collaborative drug therapy management services in designated clinics.<BR>* <BR>Provide pharmacy support of discharge facilitation and follow-up plans of care working in collaboration with other health care team members.<BR>* <BR>Lead in the development, implementation, and measurement of programs and policies related to ambulatory care and transition of care pharmacy services in collaboration with the Clinical Pharmacy Supervisor.<BR>* <BR>Provide appropriate education and staff development to pharmacy staff, medical staff, hospital staff, and clinic staff.<BR>* <BR>Service both clinical and staff pharmacist areas as needed to ensure quality pharmaceutical care to all patients.<BR><BR>Qualifications:<BR><BR>Required qualifications for this position include:<BR><BR>* <BR>Doctorate of Pharmacy (PharmD)<BR>* <BR>Current license as a Registered Pharmacist in Washington State<BR>* <BR>Completion of ASHP accredited pharmacy practice residency and ambulatory care pharmacy specialty residency or equivalent work experience in both settings<BR><BR>Preferred qualifications for this position include:<BR>* <BR>Two years of hospital pharmacy practice experience with oversight in staff education and development, leadership, and program design<BR>* <BR>Pharmacotherapy Practice board certification or Ambulatory Care Pharmacy board certification<BR>About the hospital you will serve.<BR>Providence St. Mary Medical Center in Walla Walla, Washington is a full service hospital with some of the best technology, expertise, specialists and primary care physicians in the region.
