Work Schedule: Monday through Friday 8:00AM to 4:30PM, 1st shift.
The Coordinated Community Care (CCC) Program Manager is responsible for the administrative and clinical oversight of programs aligned under CCC: Home Based Primary Care Program (HBPC), Home Telehealth (HT), Contract Adult Day Health Care Program (CADHC), Homemaker/Home Health Aide (H/HHA), Purchased Skilled Home Care, Contract Nursing Home Program, Respite, and the Non-VA Care Coordination (NVCC) programs. The Supervisory Social Worker - CCC Program Manager advises the Chief of Staff on CCC program performance, organizational risks and challenges, budget needs, and program impact(s) on patient outcomes and quality.
