Job Description: The Licensed Vocational Nurse (LVN) works under the direction of an RN and reports to the primary care manager. The LVN is responsible for observing, recording and reporting changes in the patient’s condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LVN is a member of the expanded team. The LVN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. The LVN demonstrates active collaboration and communication with Core Team (Patient, Primary Care Provider, RN Care Manager and Patient Support Technician) and Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). Provides direct nursing care to groups of patients for the Ambulatory Care area. Works under the direction of the RN, but is directly responsible to the Nurse Manager. Serves as a member of the health care team providing care on a regular and recurring basis. Utilizes knowledge and assists in patient problem assessment, making appropriate referrals, including the ability to identify emergent needs. Administers medications and collects specimens as needed. Able to work with excel documents and able to review PT medical record as well input data in the electronic dashboards, collects PT medication information and inputs changes in the computerized medical record. Able to transcribe provider notes in to the electronic medical record. Is familiar with and performs/meets Clinical Practice Guideline and Prevention Index standards within his/her scope of practice. Documentation of patient care is done electronically. Incumbent will need to process patient clinical reminders prior to their physician consultation, which are generated based on a patient’s diagnosis. Incumbent will be making various phone calls to Veterans, either to remind them of future appointment, pre appointment screening, either forwarding or providing lab results, and creating and sending reminders letter within the LVN Scope of practice.

 

Position Requirements:  Graduate of a school of Practical or Vocational Nursing, approved by the appropriate State Accrediting Agency and/or National League for Nursing at the time the program is completed. Two (2) years of successful practice as an LVN preferably in an ambulatory care setting. Excellent interpersonal skills are required along with the ability to work effectively in a diverse team environment. Flexibility and the ability to change priorities is required. Successful performance and demonstrated competencies is required along with dependability. Computer skills are essential for the job. Must have and maintain professional License and BLS Certification current.
