There is currently a vacancy for a full-time Registered Nurse as part of the Transition Team. This is multidisciplinary team of professionals who integrate, coordinate and facilitate the delivery of quality, cost-effective health care for the veteran utilizing a network of services, to include discharge planning and community services/follow up care. The nurse uses effective communication skills with patients, significant others, and all customers, internal and external to KCVA-MC. All communication is courteous, therapeutic, compassionate and professional. The tour of duty is 8:00 AM to 4:30 PM, Monday through Friday. This position reports to the Transition Team Supervisor.
