About the Position:

In this position, you will serve as a Nurse Case Manager responsible for analyzing problems associated with providing health care to mobilization/demobilization soldiers. Uses a multidisciplinary team approach to assess, plan, coordinate, monitor, and evaluate options and services to meet all mobilization/demobilization patients’ health needs. Through communication and use of available resources, promotes quality, timely, and cost effective outcomes and disposition of cases.
