THIS POSITION IS FOR A MID-LEVEL PROVIDER (NURSE PRACTITIONER OR PHYSICIAN ASSISTANT) TO PERFORM THE DUTIES AS THE HOME BASED PRIMARY CARE (HBPC) PROVIDER WHO WORKS FOR GERIATRICS AND EXTENDED CARE SERVICE.
