Position Summary:
The MHICM Clinical Nurse Specialist (CNS) is a professional nurse accountable for assisting in the selection of patient to participate in the MHICM program, for assuring that post hospital needs of the veterans are identified prior to discharge and that plans are made to meet those needs. The CNS will serve on a multidisciplinary team which provides community based case management to seriously mentally ill veterans who have a history of high hospital use. The CNS will coordinate plans for nursing interventions and prescribe psychotropic medications, and participate in multidisciplinary treatment plans for identification of problems areas and appropriate MHICM interventions. Incumbent will be responsible for completion of required clinical records as well as MHICM forms for evaluation and monitoring of service delivery and outcome: serves as a primary and secondary case manager for veterans and assesses their mental and physical status; will plan and participate in continuing educational programs and serves as a liaison between community service organizations and agencies for the MHICM program: and assists with social/recreational outing in the community and services in committees and task forces as needed. The MHICM CNS/NP is responsible to the Program Director through the Psychiatry and Nursing Services. A government car is provided for any travel involved. This is a day position, Monday through Friday, no weekends or holidays.
