The Home Based Primary Care (HBPC) Social Worker functions as part of the interdisciplinary treatment team in the HBPC program.  In addition to the assigned HBPC duties, the HBPC Social Worker coordinates the Outpatient Home Hospice Program.  The incumbent provides and coordinates health care services to veterans in their home environment.

Salary Determination: Salary is dependent upon qualifications and will be determined by a Professional Standards Board.

Appointment Information: An applicant selected for this position may be placed into a temporary appointment, not to exceed one year and one day, pending completion of the boarding process.
