About the Position:

 

The Patient Centered Medical Home (PCMH) is Army's new approach to delivering healthcare to our soldiers and their family members. The PCMH model creates medical homes, giving patients and providers the chance to get to know one another better so that personalized care plans can be developed. The PCMH practice is responsible for providing for all of a patient's health care needs or appropriately arranging care with other qualified professionals. Independent Clinical License as a Social Worker

Up to 5% Business Travel may be required.

Moving expenses will be provided.
